How to populate value in inputbox on form load from Database?

I want to fill some filed (Input boxes ) of form from database table entry. I do not find a way to achieve this.

  You need autolookup control for that.

  I use the auto lookup activity and configure my table in it. 
  But How can I configure this auto lookup activity with my input field?

  Hi Rakesh,

  The autolookup have lookups of 2 types: name/value (for dropdowns or other controls of this type) and multiple columns for other controls (for example text boxes). If you choose multiple columns you will need to check the column you want to put inside the text box and right to the column name you shoud put the internal name of the text box (drag and drop from form fields).

  Can you please give an example how to do that.I am also struggling with the same issue.

  For example, 

  Column name DB = Col1

  textbox name = Textbox1

   

  It would be great if example screenshot were share.
