Initiation issue

Has anybody ever seen a situation where you complete eform for initiation but goes to the same step? When I looked at the process it shows the green start icon checked instead of the initiation step. It's like the green start has a task of its own. I've never seen that.

  Resolved, I have auto complete turned off at the initiation step.
