Use SharePoint Lookup fields in a data grid

I have a couple of custom lists in SharePoint that have lookup columns.

I want to show some of the lookup fields in the datagrid on a  form.  None of the lookup fields are appearing in the field select. 

For example : Provision has a lookup into the Group table for name and description.  For the selected entries in the Provision table  I want to show name, userid, date, group name and group description.

  I've only managed to get this to work for the original lookup column; after some testing, I haven't been able to get supporting lookup columns to work.  It could be a bug, but I know that AgilePoint has trouble in other areas with supporting lookup columns and I've had to implement workarounds in some processes via C#, so there may be a JavaScript solution that works within a form.
